Home health care is an essential service to many patients and families. Home health care services can improve the patient's safety and overall quality of life. The patients the staff sees feel more comfortable about their safety and care.
Therapists in home health agencies provide therapy services in patients' homes. Some patients have trouble getting out to a hospital or office for therapy and prefer to receive therapy at home. In addition, therapists can provide very practical assistance in the home, like teaching a patient how to get in and out of his own bed.
Therapists assess patients in their homes to determine whether they would benefit from therapy. If a therapist determines that a patient would benefit from therapy, she devises a treatment plan to address the individual problems faced by the patient.
* To qualify for home health care, you must meet certain criterias:
1)You must be considered homebound, meaning that it is difficult for you to leave your home without considerable effort. You may need the help of a walker or a wheelchair to get around.
2) The therapy (PT, OT ,or Speech) you require must be considered medically necessary and must be ordered by a qualified physician.
3) The care must be short term typically less than eight hours a day and twenty eight hours per week of combined services for a period of up to two months.

Physical Therapists
A Licensed and Registered Physical Therapist evaluates the patient and recommends a treatment plan, if needed. This is followed by scheduled visits for the required treatment. Physical Therapists treat patients with disorders of the muscles, bones and joints along with strokes, arthritis, and nerve damage by means of physical agents: heat/cold, laser, water, electronic therapy and manual therapy (massage, myofacial release, joint manipulation) with exercises and muscle re-education.
He /She assess patients with movement problems, such as patients who have difficulty walking and prescribe exercises to treat the problem and may also prescribe assistive devices like walkers or crutches
Occupational Therapists
A Licensed and Registered Occupational Therapist evaluates and treats the patient, if needed, to improve the patient’s motor skills. Occupational therapists treat patients who have difficulty with fine motor tasks and activities of daily living. For instance, they work with patients who have trouble feeding or dressing themselves. Occupational therapists help patients improve their skills in those areas.
Speech Therapist
A Licensed and Registered Speech Therapist evaluates and treats the patient, if needed, to help him/her recover or improve his speaking abilities.
Medical Social Worker
A Licensed Social Worker evaluates patient’s needs and recommends other services that will improve the patient’s quality of life. Medical social workers assess the psychosocial functioning of patients and families and intervene as necessary. Interventions may include connecting patients and families to necessary resources and supports in the community; providing psychotherapy, supportive counseling, or grief counseling; or helping a patient to expand and strengthen their network of social supports.
